EMERGENCY CONTACT SHEET

The Parish Care Team has created an emergency contact sheet. The purpose is to assist members in contacting family and friends should an emergency occur. We often want special people with us when a crisis occurs. This information sheet will be kept confidential and shared only as directed.

Please fill out one of the sheets and return it to the church office. The form can also be downloaded here as a pdf for printing and filling out.

Our prayer is that no one will be left alone at a time when we need a friend the most.
 

This is a vital and growing ministry of Ascension Lutheran Church.  As a team, PCT seeks to provide support, educational materials, prayers, and listening ears to the congregation and community.

 A MINISTRY OF EDUCATION for body, mind, and spirit by means of:
        a) Adult Forums: medical issues (aging, osteoporosis, nutrition, depression, medication), life cycle issues (parenting, caretaking, illness, end-of-life issues), spiritual issues (grief, loss, loneliness, isolation), information about community resources
       b) A self-help library
       c) Bulletin Board; Newsletter inserts.
       d) Providing health information on an individual basis
       e) Taping Sunday services and bringing them to homebound individuals

A MINISTRY OF PRAYER by means of:
       a) The Prayer Chain.
       b) Praying with individuals
       c) Delivering the Eucharist to home or facility bound individuals

A MINISTRY OF PRESENCE by means of
       a) Follow-up home or hospital visits after an initial pastoral visit.
       b) Emotional and spiritual support.

A MINISTRY OF PROBLEM-SOLVING through linkage with community and church resources.
